We work as equal partners with our health and social care colleagues to provide social care support around 3 distinct functions.

    1. Community Bridge Building             

    Support for people with common mental health problems.The service provision will include the following:

*
 assistance in the identification and building of opportunities around an individual’s particular needs
*
   direct support and assistance necessary to make use of these opportunities, including signposting to other services
*
   assistance to access self help and support networks
*
   assistance to access employment, training and other social opportunities
*
   assistance to participate in local community activities
*
   advice about housing problems
*
   debt advice and advocacy
*
   assistance in budgeting and home maintenance skills
*
   
support to enable carer responsibilities
*
   accompanying to and from clinical sessions and other appointments
*
  emotional support to improve personal relationships
 

2. Assertive Approach

      
 Support for people with severe, ongoing mental health illness who also have difficulty engaging in lasting and consenting contact
       with Health and Social Care Services.

   

     3. Crisis Support

          
 Support for people who are users of the Crisis Services.
          Distinct focus on social context issues during crisis support period.


Frequently Asked Questions:

How to access the service? Access to the service is through agreed internal referral sources within the Community Health and Care Partnerships Glasgow i.e. Primary Care Mental Health Teams, Community Mental Health Teams and Crisis Services.

What do we mean by social care support? Central to social care services are the fundamental principals of independence, community integration, social inclusion, empowerment and choice. The service is delivered using a person centred approach. Interventions and support will focus on building a flexible and creative relationship supporting the individual emotionally, practically and socially. This may include: making lifestyle changes; addressing problems; being more able to hold responsibility for self and dependants; increase in self-reliance and resilience; more developed sense of self and others; developing emotional maturity and moving move towards being a more fully functioning person. While the services will offer a broad range of social care supports, it will not include routine personal care.
